Service Description

If you fall between the ages of 1 to 101 years old, you need (at minimum) a yearly visit with your healthcare provider, even if you have never had so much as a sore throat! We'll discuss things such as screening labs, screening tests (colonoscopy, mammograms, pap smears, etc.), and vaccines. If you have Medicare, you are eligible to receive a yearly wellness exam once every 12 months to develop or update a personalized prevention plan to help prevent disease and disability, based on your current health and risk factors. The yearly Medicare “Wellness” visit isn’t a physical exam, and a copy of the visit can be sent directly to your primary care provider (if that provider is not located at First Choice). Both of these yearly wellness checks are covered by insurance so there should be little-to-no out of pocket expense for you.
